The Personnel Action (PA) Form has been revised to provide additional detail regarding an employee’s work location. The work location in Box 29 must be selected from the drop down list for each employee’s position performed at the University to ensure accurate reporting and compliance with the City of Murray's Occupational Tax requirements. If the selected location is Other/Remote Location or Online, the city and state must also be provided. The revised version is required for all PA Forms initiated on or after the effective date of January 26, 2018.
