The Personnel Action (PA) Form has been updated to include the faculty pay plan and/or staff grade (Box 18), revisions to the selections for citizenship (Box 12), and revisions to the employment term (Box 23). The updated version should be used for any PA Forms initiated on or after the effective date of January 15, 2026.
