Murray State University has partnered with Kelly Services OCG as the Employer of Record (EOR) for U.S. Domestic Only adjunct faculty who do not have a work location in Kentucky or Illinois. The department will use this form if they have not been able to identify an instructor with a work location of Kentucky or Illinois and are requesting to use Kelly Services OCG. By completing and signing the form, the department agrees to the additional cost of using Kelly Services OCG. One form is required per course.
